PHYSICAL THERAPY – RESOURCES
BEFORE YOUR INITIAL PT VISIT

FIRST VISIT INSTRUCTIONS
Please arrive 10-15 minutes before your scheduled appointment in order to give yourself enough time to fill out the new patient paperwork that is required before treatment.
• Items that we will need from you:
o Some form of identification
o Your insurance card(s) if you choose to use it
o A form of payment (Credit card, checks accepted)
o A physician’s prescription or referral for Physical Therapy
o Any other medical records that would assist your therapist in treatment (X-Rays, MRI reports, Operative Reports, etc.)
• Please come dressed in athletic attire/comfortable clothing. We will be doing some physical activity on your first appointment and this will assist the therapist in facilitating them. We do have a locker room with showers available if you need to change!
HOW LONG ARE MY SESSIONS?
The initial evaluation of your treatment (1st visit) is typically the longest, ranging anywhere from 60-90 minutes depending on the activities performed. Each visit afterwards will be roughly 1 hour.
WHAT INSURANCES DO YOU ACCEPT?
We are currently in network with most major PPO plans on the market today.
Some examples include:
• Anthem Blue Cross
• Blue Shield (includes Covered California plans)
• Blue Cross/Blue Shield
• Cigna/American Specialty Health
• Worker’s Compensation
We are currently Out of Network with United Healthcare and Humana insurance plans. We do not accept any HMO or Medi-Cal plans.
DO I NEED A REFERRAL FROM A DOCTOR FOR TREATMENT?
While California Law allows Direct Access for Physical Therapy for 45 days or 12 visits, most insurance companies require a referral from a physician to be utilized. For this reason, Catz Physical Therapy requires a prescription from any physician to bill insurance. This referral can typically come from any physician, not just your primary. Consider an urgent care facility or any specialist physician as well!
DOES MY HEALTH INSURANCE GUARANTEE COVERAGE FOR PHYSICAL THERAPY?
Most health insurance plans consider Physical Therapy as a covered service. However, this benefit may sometimes have visit limitations or a preauthorization requirement. Our front desk staff will happily verify this information for you before you arrive for your first appointment. If you require information sooner, please consider calling the Member Services Department of your insurance plan whose number can typically be found on the back of your insurance card. Knowing your visit limitations is ultimately your responsibility, so please make sure to ask about this information before seeking treatment!
WHAT IF I DON’T WANT TO USE INSURANCE OR GET A REFERRAL?
For those not wishing to use their health insurance, we have cash rates available. Patients with high deductible plans or HMO plans could also consider using these rates to keep the cost of their treatment down. Please call the front desk at (626) 356-0599 with inquiries about the cash rate!
